2. Symptoms and Diagnoses

    The following is a list of some of the symptoms and diagnoses for which cardiac monitoring is considered medically necessary by most third party carriers:
    • Chest Pain
    • Dizziness
    • Palpitations
    • Lightheadedness
    • Face, Arm or Neck Pain
    • Arrhythmia
    • Atrial Fibrillation
    • Tachycardia
    • Heart Block
    • Angina
    • Syncope
    • Pacemaker implant

4. How To Enroll

  • Please obtain a physician's order or referral for Holter, Cardiac Event or Pacemaker Monitoring.
  • Fill out a simple Patient Information form with your address and insurance info (FCM will fax or mail this form directly to you or your physician).
  • FCM will then forward a monitor directly to your home or work (hookup to be performed over the phone with FCM technicians) OR FCM will forward a monitor directly to your physician's office for the hookup and training.
  • Call our toll-free line (1-800-274-5399) with questions on service or to transmit your EKG (open 24-hours a day, 7 days a week).
  • All reports will be faxed and mailed to your physician.
  • At the end of the monitoring period, FCM will retrieve the monitor from your home OR you may return it to your physician's office.

5. Billing Issues

  • In most cases, FCM will bill your insurance for the technical component of the procedure (Holter, Cardiac Event or Pacemaker Monitoring) while your physician will bill for the professional component of the procedure. If you do not have insurance, FCM will make arrangements with you to bill you directly for the service (payment plans are available).
  • Please obtain any authorizations or referrals as required by the terms of your insurance policy.
  • FCM is a participating provider with Medicare.
  • FCM is a participating provider with a growing list of HMO's and PPO's throughout the United States, please call our billing department for more information.
  • FCM accepts assignment from all non-managed care plans.
  • FCM accepts the following credit cards for payment of copays, deductibles, and self pays: Discover, MasterCard, Visa, and American Express.
