First-Call Medical cardiac event monitors are very easy to use for all ages. First-Call Medical technicians will assist your patients with the operation of the monitor over the telephone and will ask your patient to transmit a Baseline EKG at the time of the initial call. Here’s how to order:

To order an immediate delivery of a cardiac event monitor to your patient’s home or work, please print the Patient Information Form, have the ordering physician and patient sign the Form and fax the Form to 978-670-5457. We will contact your patient to make arrangements for immediate, FedEx delivery of a cardiac event monitor. Please be sure to specify your monitor preference: King of Hearts Express, HeartCard, or CardioMemo. All Reports will be faxed and/or mailed to your office.

• Complete the form including the diagnosis, billing information (referral or authorization if necessary), and signatures of the physician and patient
